DIRECTIONS FOR NEW CLIENTS: 

 

My office in located at 1517 Nicholasville Road in the Doctors Park across from the UK Football Field.  Once you have turned into the complex, park in front of the second building on your right.  Go through the door with the sign of "Arvil Reeb" above it.  When you enter the building, you will see my name on the inner door.  Proceed into the waiting room and have a seat.  We do not have a receptionist so you will not be checking in with anyone.  I will come out and introduce myself.  The office is handicap accessible.

 

For your convenience, I have included documents that I need for you to complete.  You may download them and bring them with you or I will have copies for you to complete once you get here.
